Immunization Information

The Cox CARE Mobile will be here at Forsyth School for your convenience on March 30th & April 1, 2022 to give immunizations and conduct sports physicals.

All 7th graders are required to have one dose of the Tdap (tetanus, diphtheria, and acellular pertussis) and one dose of the MCV (meningitis) vaccines before entering the 8th grade.  All 11th graders are required to have a 2nd MCV vaccine before entering the12th grade. If you would like your student to receive their required vaccines on the Cox CARE Mobile, please complete & return the registration/consent paperwork that was mailed to you on March 21st to the nurse's office as soon as possible and no later than March 29th.


Sports Physicals will be FREE of charge if you have Medicaid coverage, no insurance, or qualify for free or reduced lunches.  If you have private insurance, the physical will be $10 and parents must pay prior to service. 

Sports physical registration packets can be picked up at the middle school office, high school office and the nurse's office. They need to be completed and returned as soon as possible and no later than March 30th.

 Cox CARE Mobile

The Cox CARE Mobile will be here on September 26 & October 10, 2023 to do sports physicals and administer mandatory vaccines.
If you would like your student to receive either of these services, the registration/consent forms are available in the nurse's office, high school office and the middle school office.

Jordan Valley Dental Bus

The Jordan Valley Dental bus will be visiting our school on November 29th and other scheduled days in December & January, ending January 11, 2023. This program is available to students who have Medicaid.  Call the school nurse if your student is uninsured. If you would like your child to receive dental services, a registration packet can be obtained from the nurse's office, middle school office or the high school office. Appointments are limited. Priority is given to students who have the greatest dental need as well as to those who return the completed paperwork promptly.
